Application for Readmission from Suspension

This application is for an Undergraduate Student who has been academically suspended and wants to apply for readmission after sitting out for one semester or more following a first suspension, or after sitting out for two semesters or more following a second suspension. Summer semesters do not count in the time away from the university.

Graduate students who have been placed on academic suspension must go through a different process for readmission to graduate study. If you are a graduate student, please contact Graduate Studies for further information.

Deadline for application submission:

Due August 1 for Fall
Due January 1 for Spring

Any applications received after the deadline may not be considered.

Note: Failure to fully disclose all criminal charges or disciplinary actions may result in withdrawal of re-admission. In addition, failure to report any criminal charges or disciplinary actions that occur after you submit this application may also result in the cancelation of your return to the university.
