Add Form
The Add Form is used for adding courses. It is available to submit in person (with a photo ID) or via email. Please send the completed form (with signatures, only if required) to registrar@uni.edu from your UNI email. All forms received from personal (non-UNI) emails will not be accepted. Please reach out to our office via phone or email if you have any questions about which signatures are needed.
Drop Form
The Drop Form is used for dropping courses once the term has begun. It is available to submit in person (with a photo ID) or via email. Withdrawal Form
This form is utilized by a student who wishes to withdraw from all of their classes for a given term at the University of Northern Iowa. Prior to submitting the Withdrawal Form, it is strongly recommended that you read Withdrawal Schedule under the Calendars tab. You should also read Withdrawal: Refund Percentages and Grade Assignments under the Tuition & Fees tab. This form MUST be sent from your UNI email to registrar@uni.edu or submitted in person (with a photo ID).
Declaration of Curriculum
This form is used to declare or change one or more majors, minors, certificates, or pre-professional interest areas.
The department of your NEW major must approve this declaration. If you are declaring a second or third Major, departmental approval for EACH is required. If you are changing to the Exploratory Major, please visit the Office of Academic Advising (ITTC 007) for Major advisor signature.
Declaring a Minor and/or Program Certificate may require departmental approval. Please consult with the appropriate department. This form MUST be sent from your UNI email to registrar@uni.edu or submitted in person (with a photo ID).

Academic Student Request
This online form allows students to request exceptions to academic policies and regulations. A list of request types and approval steps is available here. For further questions or assistance with completing this form, please contact your academic advisor or program coordinator.
Academic Verification Request
To obtain an enrollment verification certificate now without any wait time log into MyUniverse and click on the Enrollment Verifications link in the Student Center section. This link will take you to the National Student Clearinghouse site where you can obtain certificates to verify enrollment and degrees and obtain good student discount verifications. If you have questions or need further assistance, you may fill out this form to be processed by Registrar Staff. Please allow 2 - 3 business days for processing the form.
Application for Early Readmission from Suspension
The Application for Early Readmission from Suspension is utilized by an undergraduate student who wishes to apply for early readmission at the University of Northern Iowa as a student after they have been placed on Academic Suspension.

The Application for Readmission from Suspension is utilized by an undergraduate student who wishes to return to the University of Northern Iowa as a student after they have been placed on Academic Suspension.
Audit and Credit/No Credit Request
The Audit and Credit/No Credit Request is to be utilized by undergraduate students who would like to change a course from a graded basis to an audit basis, or who would like to change a course from graded basis to a credit/no credit basis.
Change of Contact Information Form
The Change of Contact Information Form is to be utilized by students who would like to update their contact information. Once this form is filled out and signed, it should be submitted to the Office of the Registrar for processing.
Credit by Exam Application
Credit by examination is for UNDERGRADUATE STUDENTS ONLY. Submit the completed application form with the necessary approvals to the Office of the Registrar. A student should discuss satisfying a course requirement with their academic advisor and with the department head (from the department offering the exam) before applying for credit by examination. Declaration of Curriculum

Directory Information Change
This form can be used to restrict the University from releasing Directory Information items to outside parties. Unless otherwise noted, all Directory Information items will be made available upon request. For more information about FERPA and Direction Information items, please see our FERPA webpage.
Early Degree Conferral Form
This Early Degree Conferral Form indicates that you would like to be awarded your degree at the time of session completion, rather than waiting until the end of the academic term. Forms must be submitted to the Office of the Registrar for processing. Forms from personal email addresses will not be accepted.
Extenuating Circumstances Tuition and Fees Refund Request
This form is utilized by students who are wanting to file an exception to the Tuition and Fees Refund Policy due to extenuating circumstances. This form must be filled out and submitted to the Office of the Registrar with documentation of the extenuating circumstance.
Medical Documentation Form
The Medical Documentation Form is utilized by a student who is petitioning for exceptions to University policy.
Name Change Form
Students can utilize this form to change how their name is listed in the University's student information system. The name on the student record should be the student's complete and legal name. Once this form is filled out and signed, it should be submitted to the Office of the Registrar for processing.
Re-Entry Form
This form is to be utilized for students that have previously attended the University of Northern Iowa but have not obtained a Bachelor's-level degree and would like to return to UNI.
Residency Classification Application
This form is utilized by a student who wishes to request a change in their residency classification. Before we can process the Resident Classification Application, this form must be filled out, signed, and returned to the Office of the Registrar along with any supporting documentation. Prior to submitting the Residency Classification Application, it is strongly recommended that you review the General Information Regarding Residency page under the Students tab.

Add Course Section
The Add Course Section is utilized by departmental schedulers to add a course to the Schedule of Classes.
Ad Hoc Report Request
The Ad Hoc Report Request is utilized by students, faculty, staff who are in need of information that is not already provided elsewhere by the university. Prior to filling out and submitting this WebForm, please take a moment to review the information on our Reports webpage and also on the Office of Institutional Effectiveness and Planning website.
Change Course Section
The Change Course Section Information is utilized by departmental schedulers to change section information for a course in the Schedule of Classes.
CIEP Registration
The CIEP Registration form is used by the Cultural & Intensive English Program to submit registration changes for students enrolled in the program.
Class Roster Verification Form
The Class Roster Verification Form is utilized by instructional staff to verify class registration. Please compare your current Class Roster(s) in MyUNIverse with the students actually attending class. If there are discrepancies between Class Roster(s) and student attendance, please note them on this form. If there are courses in which students may earn differing amounts of credit, please note this in the "Hours Credit" area. If there are no discrepancies to report, you do not need to submit this WebForm.
Combined Section Request Form
This form is utilized when departments are seeking to combine two sections in the Schedule of Classes for registration purposes. This webform is considered a request to combine sections. You will be notified if it meets the criteria for combining.
Drop Course Section
The Drop Course Section WebForm is utilized by departmental schedulers to change a course from the Schedule of Classes. This is not for students intending to drop a course.
Experimental Course Form
This form should be utilized to set up a new experimental course at the Academic Catalog level, so that it may be scheduled. A separate form must be completed each semester the course is offered on an experimental basis.
Faculty & Staff Commencement Ceremony Participation WebForm
The Faculty & Staff Commencement Ceremony Participation WebForm is utilized by faculty and/or staff members who wishes to participate in a Commencement Ceremony.
Hybrid Course Request Form
The Hybrid Course Request Form is intended for departments wishing to schedule hybrid classes due to COVID-19 planning. Please complete this webform if you wish for some students to attend one day per week and the remaining students on another day. For example, if a course was scheduled to meet 8am TR and the desire is for 50% to attend Tuesday and the other 50% to attend Thursday, this form would need completed to create a new section.
Instructor/Advisor Set-Up Request Form
The Instructor/Advisor Set-Up Request Form is used by departmental schedulers to request set-up of a new faculty under a specific Academic Organization. The Schedule Change Request - Change Course Section Information WebForm will also be required to assign the instructor to a specific course.
New Course Title Request
This form is intended to be used for Studies in, Seminars, or Other Courses with Course Titles/Topics as indicated in the Schedule of Classes. Prior to submitting this form, it is strongly recommended that you review the information on the Call for Classes for the appropriate term.
Request for Variance Form
Any request for a variance in standard class meeting times must be made using the Request for Variance Form. This form will then be routed to the Department Head, Dean, and finally to the Office of the Provost and Executive Vice President for a final decision. Your form must clearly articulate why such a variance should be approved. Please note that a Variance Form is required every term for any courses falling outside of the Standard Class Meeting Time policy, it is not guaranteed that a course approved for a variance in prior terms will be approved for another future.
Request to Change a Room Due to a Disability
It is recommended that accommodation requests for room assignments take place prior to the creation of the schedule of classes, or as soon as practicable, to ensure completion prior to the start of classes. Requests for spring semesters should be made by October 1, summer requests by February 1 and fall requests by March 1. Any late occurring need for an accommodation request for an upcoming semester or summer term should be made as soon as practicable after the need arises.

Course Description Request Form
Course descriptions from 2012 to present are available on the Academic Catalog web page. If you are requesting a course description prior to 2012, please fill out the Course Description Request Form. If you are in need of a course syllabus, please contact the appropriate departmental office.
Diploma Reorder
Use the Credential Order Form to submit your order for a traditional printed, duplicate copy of your diploma or a Legacy CeDiploma (certified electronic diploma).
Please be sure to select the correct credential type from the Available Product drop-down menu:
- Print Duplicate Diploma, (all graduates): $35.00
- Legacy CeDiploma (graduates fall 2023 and earlier): $70.00 (A certified electronic diploma, that cannot be printed.)
Please allow 10 working days + delivery time, for your diploma to be printed and mailed to you. International orders may take longer.
- Your name on the diploma will appear as it does on your UNI student records unless you submit the Name Change Form with the required documentation to support the name change.
- All duplicate diplomas will have the notation "Duplicate diploma issued" with the issue date in the lower left corner. All signatures will reflect current university and state officials.
- A duplicate copy of a diploma will not be issued to former students whose University account is restricted for past due U-Bill payment.
- If sent to a domestic (U.S.) address the diploma will be sent through US Postal Service. If being sent to an International address, it will be sent by USPS International Air Mail with a tracking number.
If your diploma was damaged or destroyed due to a natural disaster, fire, home damage, or some other extenuating circumstance, please contact us at commencement@uni.edu prior to submitting a Diploma Reorder Form.
MPLS Transcript Request
The MPLS Transcript Request form may be completed by a Malcolm Price Laboratory School student or their parent and/or guardian.
Subsequent Major Form
This form is to be utilized by students who have already completed a UNI Baccalaureate degree and are seeking approval to complete an additional major. In addition, a Non-Degree Application for Admission will need to be completed. If 32 or more hours are needed to complete the subsequent major or if it would lead to a different degree (i.e. Bachelor of Science), the student must apply through the Office of Admissions as a Post-Baccalaureate student.
Transcript Request
This form is utilized by an individual who wishes to reorder a university transcript and is unable to access the preferred Parchment request.

Classroom Reservation Request
The Classroom Reservation Request is utilized by students, faculty, and/or staff who would like to request the use of a general classroom for an academic class, such as an additional meeting time, an alternate (or secondary) exam room, a break out room, review sessions, class presentations, or as a space for student organization meetings. Iowa Voter Registration Application
This form is utilized by an individual who wishes to register to vote in the State of Iowa. It is important to note that the individual utilizing this form must be a United States citizen and 18 years of age or older on or before the day of the election in which they intend to participate. For more information related to the use of this form, please refer to the Iowa Secretary of State or to the Voter Registration webpage on our website.
National Mail Voter Registration Application
This form is utilized by an individual who wishes to register to vote. It is important to note that the individual utilizing this form must be a United States citizen and 18 years of age or older on or before the day of the election in which they intend to participate. For more information related to the use of this form, please refer to the U.S. Election Assistance Commission or to the Voter Registration webpage on our website.
